Release and Waiver of Liability
I am participating in yoga classes, workshops, training, and/or other wellness and healing arts activities offered by Kundalini PHX. The activities may be offered in the physical location of Kundalini PHX, in another host site, or offered online through videos, podcasts, apps, or other digital media or platforms.
I understand that the opinions, services, and instruction offered to me by Kundalini PHX or other instructors at their request are NOT medical advice, consultation, therapy, or treatment, nor do they include any other medical, mental, physical, or spiritual diagnosis. I understand that the educational services and instruction provided by the instructors are not intended to be in place of or instead of any medical advice or treatment that I may require for any cause whatsoever, now or in the future.
I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the individual/group classes/workshops offered. I represent and warrant that I am physically fit and have no medical or mental condition that would prevent my full participation in classes, workshops, or instruction.
I recognize that yoga requires physical exertion, which may be strenuous and cause physical injury, and I am fully aware of the risks and hazards involved. I specifically agree that Kundalini PHX, all instructors, and any or all subordinate or affiliated people or organizations shall not be liable for any claim, demand, or cause of any action of any kind, whatsoever for, on account of death, personal injury, property damage or loss of any kind resulting from or related to my use of the facility or participation in any exercise, yoga, or activity within or without the facility premises.
I understand that my behavior in class must be appropriate. Otherwise, I may be asked to leave with no refund issued.
I understand that Kundalini PHX may photograph, video, or otherwise record classes or events occurring at Kundalini PHX and place such photographs and videos on its website or social media platform. I consent to Kundalini PHX's use of my image that may appear in any such photograph or video.
I, my heirs, or legal representatives forever agree to release and discharge all instructors, employees, officers, and agents of Kundalini PHX and their assigns from any claims or causes of action known or unknown arising out of any act or negligence arising from my participation in their courses.
